8:06 During fasting or starvation ; gluconeogenesis takes place in the majority of tissues through 2 main pathways : (1) Oxidation reaction of lactate to pyruvate . (2) Transamination reactions of amino acids released from protein breakdown to their corresponding alpha keto acids . Pyruvate - an alpha keto acid - generated either by oxidation of lactate or transamination of alanine amino acid undergoes a key step in the gluconeogenesis pathway which involves its CARBOXYLATION to oxaloacetate ; a process catalyzed by the enzyme PYRUVATE CARBOXYLASE . Pyruvate carboxylase enzyme is highly regulated by ATP levels & ALLOSTERICALLY ACTIVATED by acetyl coA levels . So lack of ATP & acetyl coA will down regulate its activity . In case of medium chain acyl coA dehydrogenase (MCAD) deficiency , the ATP & acetyl coA production during fasting , starvation & illness is markedly reduced . As a result the generation of oxaloacetate from pyruvate & the subsequent production of glucose from oxaloactate down that gluconeogenic pathway will be markedly reduced ; leading to HYPOGLYCEMIA ! 2 points are worth mentioning : 1. Medium chain & short chain fatty acids do NOT utilize the carnitine shuttle across the mitochondrial membranes to reach the mitochondrial matrix ; only the long chain fatty acids do . 2. In aerobic conditions ; for every single acetyl coA molecule produced from glucose through glycolysis & pyruvate oxidative decarboxylation an energy equivalent of 6 ATP molecules is generated ; while in case of beta oxidation of fatty acids only 4 ATP molecules are generated for every single acetyl coA molecule with an overall subtraction of 2 high energy bonds consumed during the initial step of activation of the fatty acid to acyl coA . So glucose is a way energy efficient compared to fatty acids & less amount of oxygen is required to generate the vital energy needs of the body through glucose compared to fatty acids requiring higher oxygen consumption ! In a way or another ; that might explain the pathognomonic sign of ketosis , Kaussmaul sign of rapid deep respiration OR hyperventilation when beta oxidation of fatty acids takes the upper hand . Lord knows best !

I can’t believe other people are talking about this. I was diagnosed with CPT II around 17 years old and now that I’m older age 38 it seems to be getting worse. I get random swelling and pain in my foot and my elbow and sometimes my legs. And yes, it can be unbearable to a point that I have to use crutches to get around. Not many people know about this because it’s so rare. Any tips you guys may have is greatly appreciated.
